Purpose:
This module will help you carefully prepare your application for a care level assessment or an increase in your care level. Together, we will structure all the important steps, clarify which documents are required, and create clear, comprehensible information for a complete and well-organized application. The goal is for you to understand the process, gain confidence, and be able to document your situation clearly.

Important Note:
I cannot influence or guarantee the decision of the long-term care insurance provider. However, I will support you in submitting a well-prepared, complete, and clear application.

In this module, we focus on structuring and sorting your documents so that you are well prepared for applications and appointments related to care – with exactly the information that is truly necessary.

  • Sorting and sensible filing of applications and appointments
  • Compilation of the most important documents for care level, benefits and discussions
  • Explanation of basic nursing terms you should know for applications and appointments
  • Creating a clear list of what you need to take with you, when, and where.

To ensure an efficient on-site appointment and adherence to the agreed time, please have the relevant documents ready.

Data protection and confidentiality during on-site appointments:
During on-site appointments, I maintain strict confidentiality. Your private documents will only be viewed within the scope of the agreed-upon service and will neither be stored nor shared. Before each appointment, we will jointly sign a written confidentiality agreement to protect your data and documents.

  • Setting the frequency: Weekly (intensive), monthly (standard) or annually (monitoring)

  • Handover of a customizable plan (PDF/checklist) with flexibility for changes​
  • Reminder service – Reliable support in the background

Purpose: Flexible long-term package for ongoing structure – the customer determines what is done and when, without rigid modules. Ideal for ongoing care with varying needs.​
